If you are over the age of 16 and are registered with a GP in Leeds, Linking Leeds can enable and empower you to make a change. A wellbeing coordinator will support you to explore the different areas of your life and create an action plan. Once your goals are set, we can link you to other professional organisations, community groups and one to one services/activities to help you achieve your goals working with you for up to 12 weeks.
About the service
Linking Leeds is the integrated city-wide Social Prescribing service for people in Leeds.
Community Links is leading a consortium of seven local partners to deliver this new service from the 1st of September 2019, commissioned by NHS Leeds CCG. Our partnership has more than ten years of collective experience delivering Social Prescribing to the people of Leeds.
The new service will see consistent Social Prescribing for all people aged 16 years and above, no matter where they live in the city – while responding to the specific health profiles of communities.
Aims of the service
Our aim is to connect people to services and activities in their community in order to benefit overall health and wellbeing. We believe that everyone can live a fulfilled life, however, sometimes people need help to achieve this. Feeling linked to your community has lots of health and wellbeing benefits that support resilience, independence and good physical and mental health.
Our Wellbeing Coordinators are based within GP practices providing one-to-one support over the phone and face to face. Community outreach from GP surgeries and other community locations is undertaken as appropriate.

GP
An appointment with one of our experienced Wellbeing Coordinators can be booked in directly through your GP.
Telephone
All telephone introductions should be made to the Linking Leeds Hub on 0113 336 7612.
